February 15, 2024 - In follow up to the communication that was sent on October 5, as per the Provincial Informed Consent Guidelines and Manitoba’s Seasonal Influenza and COVID-19 Program Plan 2023-24, verbal and/or written consent must be obtained prior to immunization and must be documented via a consent form, medical chart or electronic health record. If choosing to document consent using a paper consent, Manitoba Health provides a combined influenza, COVID-19, and Pneu-P-23 Vaccine Consent Form which is available online at:
- Flu website
- COVID-19 website
If medical clinics and pharmacies want to use their own consent form, please ensure it meets the minimum requirements as outlined in the Provincial Informed Consent Guidelines and the data collected meets the requirements outlined in the Publicly Funded Provincial Immunization Program Standards.
