Newcomer to Canada Form Adobe Reader
In addition to completing the Manitoba Prenatal Benefit form, newcomers to Canada who have not filed an income tax return must complete this form and provide required documents.

Change in Employment of Family Status Adobe Reader
This form must be completed by applicants who have a current year income that is significantly less than their net family income from the previous taxation year.

Declaration of Income for Minor Applicants Adobe Reader
In addition to completing the Manitoba Prenatal Benefit application form, applicants 18 years and under, who have not filed an income tax return must complete this form.  Applicants who do not have a Social Insurance Number (SIN) must also include a completed copy of the SIN application form.
